If you want to request a copy of your medical records at Shore Medical Center, please call the Health Information Management Services (HIMS)
Department at 609-653-3613 Hours: Monday-Friday 7:30am to 4pm.
NOTE: There may be a 36 hour delay for posting most information.
If you think information is missing from your record please know, there may be a 36 hour delay for posting most information. If you want to request
records from your physician, please contact your doctor’s office.
If you haven’t received your Welcome email, Validation Key code, Need password reset, or if you are experiencing an issue connecting to the
patient portal please call Shore Medical Center’s Health Information Management Services (HIMS) Department at 609-653-3613 Hours: Monday-Friday 7:30am to 4pm.

If you forgot your password, go to the log-on screen, enter your username and click the ‘Forgot your Password’ link. Enter the answers to
your three security questions and reset your password.
